چکیده

Background FFR was initially validated against SPECT, Dobutamine Stress Echo and Exercise Testing [1]. It is now frequently used to determine the management of intermediate coronary artery stenosis. A cut-off value of 0.75 is used in clinical practice to guide revascularisation supported by long-term outcome data [2], but a ‘grey zone’ of 0.75-0.8 with uncertain clinical significance exists [3]. Advances in non-invasive imaging tests (gated SPECT and CMR) warrant a re-evaluation of FFR at intermediate stenosis severity against non-invasive imaging.

Deconstructing the idol of fractional flow reserve using the IDEAL report.

Percutaneous coronary intervention (PCI) in patients with chronic stable coronary artery disease (CAD) has not yet been proven to reduce the incidence of myocardial infarction or death. The outcome after PCI is, however, dependent on the amount of myocardial ischaemia, which is—among others—influenced by the tightness and proximity of the coronary atherosclerotic lesion. Invasive ischaemia test...

Assessment of the contractile reserve in patients with intermediate coronary lesions: a strain rate imaging study validated by invasive myocardial fractional flow reserve.

AIMS The present study aims to compare the change of left ventricular deformation during dobutamine stress echocardiography (DSE) with the reference standard of invasive myocardial fractional flow reserve (FFR) to assess the haemodynamic significance of intermediate coronary lesions. METHODS AND RESULTS In 30 patients with an intermediate coronary artery stenosis in one epicardial coronary ar...

A study of noninvasive fractional flow reserve derived from a simplified method based on coronary computed tomography angiography in suspected coronary artery disease

BACKGROUND The invasive fractional flow reserve has been considered the gold standard for identifying ischaemia-related stenosis in patients with suspected coronary artery disease. Determining non-invasive FFR based on coronary computed tomographic angiography datasets using computational fluid dynamics tends to be a demanding process. Therefore, the diagnostic performance of a simplified metho...

Non-Invasive Fractional Flow Reserve Estimation with Coronary Computed Tomography Angiography

Coronary artery disease (CAD) remains a significant cause of morbidity and mortality worldwide [1]. Myocardial ischemia results when myocardial blood flow is insufficient to meet demand; this causes symptoms and affects patient outcome [2]. According to current guidelines for the management of stable CAD, revascularisation should be guided by the presence of myocardial ischaemia. The treatment ...

Cardiac magnetic resonance perfusion imaging for the functional assessment of coronary artery disease: a comparison with coronary angiography and fractional flow reserve.

AIMS Cardiac magnetic resonance perfusion imaging (CMRI) is a promising technique for non-invasive measurement of myocardial perfusion reserve. Fractional flow reserve (FFR) is an established invasive method for functional assessment of coronary artery disease (CAD). To prospectively assess the diagnostic value of CMRI for the detection of haemodynamically significant coronary lesions, compared...
